When is it advisable to attend a travel clinic before an international trip?

Planning a trip also means making provisions for certain health-related matters. Attending a pre-travel consultation several weeks in advance allows you to review your existing vaccinations, complete any necessary vaccination schedules, assess other preventive measures and tailor the recommendations to your itinerary and personal circumstances. As a general guide, it is advisable to do this between four and eight weeks before departure, although some trips may require planning further in advance.

As a trip approaches, almost everything seems to have a clear deadline: buying tickets, checking your passport, finalising the itinerary, packing your bags. Health preparations, on the other hand, are often left until the last minute.

It is common for people to book a consultation just a few days before setting off, often because they see it primarily as a question of: ‘Do I need any vaccinations?’ However, travel medicine covers much more than that. Before recommending any preventive measures, it is important to find out what protection the person already has, what their actual itinerary will be, how long they will be away, and whether there are any medical circumstances that need to be taken into account.

That is why timing matters.

As a general guideline, travel health recommendations suggest scheduling the consultation between four and eight weeks before departure. This is not a strict countdown, but rather a reasonable timeframe to allow for review, decision-making and, where necessary, the completion of certain procedures.

Dr Abelardo Fernández Chávez, a specialist in Preventive Medicine and Public Health and head of the International Traveller’s Clinic at the Publio Cordón Memorial Hospital, explains that this timeframe should be understood as more than just ‘time to get vaccinated’:

‘Consulting several weeks in advance allows us to review the protection the person already has, complete vaccination schedules where necessary and calmly assess other preventive measures. Some trips can be prepared in a single consultation; others require a little more time.’

Why might several weeks be necessary?

Because preparing for a trip from a health perspective cannot always be completed in a single visit.

The first step is usually to review previous vaccinations. For some people, it will be sufficient simply to confirm that their vaccination schedule is up to date. For others, there may be outstanding doses, a need to complete a schedule started previously, or a recommendation for a vaccine related to the nature of the trip.

And each vaccine has its own timing requirements.

Some require several doses administered at specific intervals. Others need a certain period to elapse after administration to achieve the expected protection or for specific documentation to be valid. Having a buffer allows these times to be organised without relying unnecessarily on the last few days.

For Dr Fernández Chávez, reviewing a patient’s medical history before adding new vaccines is an essential part of the consultation: ‘Before recommending a vaccine, we need to know what the traveller has already received. This enables us to identify incomplete vaccination schedules, assess whether any additional doses are required, and avoid interventions that offer no benefit’.

In specific situations, there may also be uncertainties regarding previous vaccinations, medical history or treatments, making it advisable to review records or carry out further assessments. This does not mean that all travellers need laboratory tests or serological tests: tests must always be based on a specific medical indication.

Planning ahead allows these issues to be resolved in good time and helps to avoid hasty decisions.
